Team Leader, Health & Social

This role is for a Team Leader in a health and social care setting, offering 37.5 hours per week. The position is based on the Isle of Man, and candidates must either be residents or hold a work permit at the time of appointment. The Team Leader will play a crucial role in meeting the practical, physical, and emotional needs of the individuals within the service. They will design individual support programmes through a key worker system and coordinate the delivery of these packages alongside a team of Support Workers. Responsibilities include planning the transition of new individuals into the service, advocating for their needs, and ensuring that all interactions uphold dignity, respect, and choice. The Team Leader will also manage compliance with regulatory bodies and maintain accurate records according to organisational policies. Additionally, the role requires effective leadership and development opportunities for both the Team Leader and Support Workers, all while addressing the unique needs of each individual.

Job Duties:

  • Plan, coordinate, and implement the safe transition of new individuals into the service and community as needed.
  • Communicate with families and relevant members of multi-disciplinary teams to advocate for the needs of individuals.
  • Develop and implement individual and group support programmes to enhance independence.
  • Coordinate regular assessments and reviews using appropriate assessment models.
  • Address the physical, emotional, and personal needs of individuals, ensuring a focus on individual requirements.
  • Ensure medication is managed according to policy and current statutory procedures.
  • Provide leadership and management to Support Workers, including attendance monitoring and involvement in capability, disciplinary, and grievance procedures as necessary.
  • Participate in learning and development opportunities while identifying development needs for self and Support Workers.
  • Maintain adequate records and produce necessary reports in compliance with organisational requirements.
  • Assist the manager with compliance concerning relevant regulatory bodies.

Job Requirements:

  • Minimum of 2 years’ experience in a health and social care setting.
  • Currently enrolled in QCF Level 3 in Health and Social Care or equivalent.
  • Understanding of the mental health/learning disability sector.
  • Qualifications in QCF/RCF Level 3 in Health and Social Care, Social Work, First Level Nursing, Allied Health Profession, or a Level 5 Diploma in Leadership for Health and Social Care (adults management/adults residential management, Wales & NI).
  • Full valid driving licence and access to own vehicle.
  • Strong verbal and written communication skills.
  • Report writing, assessment, and evaluation skills.
  • Competency in risk assessment and organisational skills.
  • Availability to work flexibly.
